For the best-known combination, dasatinib plus quercetin (D+Q), the lifespan evidence is in mice; the first human trial was a small feasibility study in a lung disease, with no lifespan or aging endpoint.\n\nMICE (Xu et al., Nature Medicine 2018). Species: mice. Intervention: intermittent oral D+Q, started very late in life. The paper reports that \"mice with bi-weekly administration of D+Q starting at 24-27 months of age (equivalent to age 75-90 years in humans) had 36% higher median post-treatment lifespan\" and \"lower mortality hazard\" \"compared to the vehicle group\". The abstract puts it as \"increased post-treatment survival by 36% while reducing mortality hazard to 65%\", i.e. a hazard ratio of about 0.65, or roughly 35% lower risk of death at any point (100% minus 65%). Note the endpoint: this is survival measured from the start of treatment in very old mice, not whole-life lifespan. Group sizes for the lifespan arm sit in a figure legend that is not in the stored text, so I'm not quoting them here.\n\nOne human-tissue element in the same paper is easy to misread as a human result: D+Q \"decreased the number of naturally occurring senescent cells and their secretion of frailty-related proinflammatory cytokines in explants of human adipose tissue\". That is fat tissue in a dish, not people taking the drug. The authors' own conclusion is scoped to mice: \"senolytics can enhance remaining health- and lifespan in old mice\".\n\nHUMANS (Justice et al., EBioMedicine 2019, NCT02874989). Design: \"A two-center, open-label study of intermittent DQ\", dosed \"D:100 mg/day, Q:1250 mg/day, three-days/week over three-weeks\". Population and size: it \"was conducted in participants with IPF (n = 14) to evaluate feasibility of implementing a senolytic intervention\" (IPF is idiopathic pulmonary fibrosis). Phase: a first-in-human pilot with no placebo or control arm. Primary endpoint: \"The primary endpoints were retention rates and completion rates for planned clinical assessments\", meaning feasibility, not efficacy. Safety: \"One serious adverse event was reported\". Secondary results: \"Physical function evaluated as 6-min walk distance, 4-m gait speed, and chair-stands time was significantly and clinically-meaningfully improved (p < .05)\", but \"Pulmonary function, clinical chemistries, frailty index (FI-LAB), and reported health were unchanged\". The abstract gives p-values, not effect sizes, for the function gains. The authors call for \"warranting evaluation of DQ in larger randomized controlled trials for senescence-related diseases\".\n\nWhere that leaves the claim:\n1. A lifespan effect of D+Q is shown only in mice, and only as post-treatment survival from 24-27 months of age (HR about 0.65).\n2. In humans, the evidence here is 14 people with IPF, 3 weeks of dosing, no control group, a feasibility primary endpoint, and improved walking and chair tests while lung function and a frailty index did not change.\n3. Nothing in either paper measures human lifespan or a general aging outcome. \"Senolytics extend life\" is a mouse statement.\n\nLimits: abstracts plus one located passage of the mouse paper's full text. Later randomized senolytic trials exist and aren't covered here.","topics":["global-health","medicine"],"evidence":[{"end":1244,"start":1165,"source_id":"MED:29988130","doc_sha256":"77fa07b052030dca8992e44431beaba6cd97e7cac702539ba1121a840f18698a","excerpt":"increased
