The best primate data and the best human trial answer different questions.\n\nMonkeys (Mattison et al., Nat Commun 2017): this paper directly compares the two rhesus monkey studies begun in the late 1980s. Even in primates, its impact \"has been controversial\": \"The University of Wisconsin study reported a significant positive impact of CR on survival\", while \"the National Institute on Aging study detected no significant survival effect\". The authors attribute part of the gap to design, and \"we report species specificity in the impact of CR in terms of optimal onset and diet\". Their conclusion is about health benefits, and they \"suggest that CR mechanisms are likely translatable to human health\". That is a suggestion, not a measured result in humans.\n\nHumans (Kraus et al., Lancet Diabetes Endocrinol 2019, CALERIE, NCT00427193): \"CALERIE was a phase 2, multicentre, randomised controlled trial\" in \"young and middle-aged (21-50 years), healthy non-obese\" adults, randomized 2:1 to \"a 25% calorie restriction diet (n=143, 66%) or an ad libitum control diet (n=75, 34%)\". People didn't hit the target. The CR group \"achieved a mean reduction in calorie intake of 11·9% (SE 0·7; from 2467 kcal to 2170 kcal)\", less than half the prescribed 25%, with a \"sustained mean weight reduction of 7·5 kg\". \"Calorie restriction caused a persistent and significant reduction from baseline to 2 years of all measured conventional cardiometabolic risk factors\", including LDL cholesterol and blood pressure. The paper labels these outcomes exploratory.\n\nThree gaps:\n1. Endpoint: survival (mixed across two monkey studies) versus 2-year risk factors in humans. CALERIE measured no lifespan, mortality or disease events.\n2. Dose: the monkey studies imposed CR; in people, ~12% was what a prescribed 25% actually delivered.\n3. Population: CALERIE enrolled healthy, non-obese 21-50 year olds, so it says nothing directly about older adults.\n\nReading it: \"CR improves cardiometabolic risk markers over 2 years\" is supported in humans by a randomized trial. \"CR extends lifespan\" isn't settled even in rhesus monkeys, and these sources have no human lifespan data.\n\nLimits: abstracts only. Other CALERIE analyses, such as biological-age measures, aren't covered here.","topics":["global-health","medicine"],"evidence":[{"end":183,"start":129,"source_id":"MED:28094793","doc_sha256":"19e6cad63384b56447152a5122887eb59a155185dde07e89a916c95631bf491e","excerpt":"its impact in nonhuman primates has been controversial","source_title":"Caloric restriction improves health and survival of rhesus monkeys.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":378,"start":288,"source_id":"MED:28094793","doc_sha256":"19e6cad63384b56447152a5122887eb59a155185dde07e89a916c95631bf491e","excerpt":"The University of Wisconsin study reported a significant positive impact of CR on survival","source_title":"Caloric restriction improves health and survival of rhesus monkeys.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":461,"start":384,"source_id":"MED:28094793","doc_sha256":"19e6cad63384b56447152a5122887eb59a155185dde07e89a916c95631bf491e","excerpt":"the National Institute on Aging study detected no significant survival effect","source_title":"Caloric restriction improves health and survival of rhesus monkeys.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":811,"start":727,"source_id":"MED:28094793","doc_sha256":"19e6cad63384b56447152a5122887eb59a155185dde07e89a916c95631bf491e","excerpt":"we report species specificity in the impact of CR in terms of optimal onset and diet","source_title":"Caloric restriction improves health and survival of rhesus monkeys.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":969,"start":903,"source_id":"MED:28094793","doc_sha256":"19e6cad63384b56447152a5122887eb59a155185dde07e89a916c95631bf491e","excerpt":"suggest that CR mechanisms are likely translatable to human health","source_title":"Caloric restriction improves health and survival of rhesus monkeys.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":449,"start":386,"source_id":"MED:31303390","doc_sha256":"f8c31e8fd1803f45370ef924c7029936f6165e9900ffddc6d51bba3b873b9ed3","excerpt":"CALERIE was a phase 2, multicentre, randomised controlled trial","source_title":"2 years of calorie restriction and cardiometabolic risk (CALERIE): exploratory outcomes of a multicentre, phase 2, randomised controlled trial.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":507,"start":453,"source_id":"MED:31303390","doc_sha256":"f8c31e8fd1803f45370ef924c7029936f6165e9900ffddc6d51bba3b873b9ed3","excerpt":"young and middle-aged (21-50 years), healthy non-obese","source_title":"2 years of calorie restriction and cardiometabolic risk (CALERIE): exploratory outcomes of a multicentre, phase 2, randomised controlled trial.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":1452,"start":1367,"source_id":"MED:31303390","doc_sha256":"f8c31e8fd1803f45370ef924c7029936f6165e9900ffddc6d51bba3b873b9ed3","excerpt":"a 25% calorie restriction diet (n=143, 66%) or an ad libitum control diet (n=75, 34%)","source_title":"2 years of calorie restriction and cardiometabolic risk (CALERIE): exploratory outcomes of a multicentre, phase 2, randomised controlled trial.","source_provenance":"registry","source_url":null,"source_origin":null},{"end":1589,"start":1499,"source_id":"MED:31303390","doc_sha256":"f8c31e8fd1803f45370ef924c7029936f6165e9900ffddc6d51bba3b873b9ed3","excerpt":"achieved a mean reduction in calorie intake of 11·9% (SE 0·7;
