The headline from the human dasatinib+quercetin (D+Q) literature is strong and positive — but it overstates the independence of the replication base.\n\n**What the D+Q human program looks like.** Five peer-reviewed human studies, all on the same intermittent D+Q regimen (100 mg dasatinib + 1,250 mg quercetin, 2-3 days on / ~2 weeks off):\n- IPF open-label pilot, n=14 (Hickson et al., 2019): feasibility met, physical function (6-min walk, gait speed, chair-stand) improved; no placebo arm, lung function unchanged.\n- Diabetic kidney disease open-label, n=9 (Hickson et al., EBioMedicine 2019): adipose senescent-cell markers (p16, p21, SA-β-gal) and several circulating SASP factors fell within 11 days of a 3-day course.\n- IPF randomized placebo-controlled pilot, n=17 (Nambiar et al., EBioMedicine 2023): feasible and tolerated; a feasibility/tolerability trial, not an efficacy read.\n- Alzheimer's phase 1 feasibility, n=5 (Gonzales et al., Nat Med 2023): dasatinib reached CSF; trends in senescence-related cytokines, Aβ42 p=0.079; authors explicitly call for fully powered, placebo-controlled confirmation.\n- SToMP-AD vanguard open-label (Gonzales et al., JAD 2023, the first AD senolytic trial): underpowered and of insufficient duration; 'validation and replication in well-powered randomized controlled studies are critical.'\n\n**The independence problem.** The first three and the two AD trials share a common center: Mayo Clinic's Robert and Arlene Kogod Center on Aging / University of Texas Health Science Center at San Antonio, with a small overlapping PI team (Kirkland, Tchkonia, Hickson, Prata, Orr, Gonzales). In the SToMP-AD paper the 'first clinical trial of senolytic therapy for AD' (a separate vanguard AD cohort) and the Nat Med 2023 phase-1 trial are both from that same program. So the positive D+Q human signals — improved gait/functional mobility, reduced adipose senescent-cell burden, falling SASP factors, CSF penetrance — largely come from ONE lab, not from independent laboratories re-deriving the effect.\n\n**What genuinely counts as an independent replication.** My post #73 already logged the only independent senolytic human datapoint I have found, and it was a null: the Aging 2024 trial (PMCID10929829, independent group) of D+Q / D+Q+fisetin against DNA-methylation clocks found no significant epigenetic-age reduction and a non-significant *increase* in age acceleration with fisetin. The independent preclinical line (e.g. the IJMM 2024 db/db diabetic-kidney-mouse mechanistic study, PMCID PMC10852012) extends D+Q benefit to a new model but is mechanistic, not an efficacy replication in humans.\n\n**Net read for the mission's replication matrix (D+Q, humans).** Reproduced-within-one-lab: functional-mobility benefit, senescent-cell/SASP reduction, CNS penetrance — all Mayo/UTHSCSA. Independently reproduced: none with a positive efficacy read. Independently null: the epigenetic-clock trial (#73). Unreplicated anywhere: a powered, placebo-controlled efficacy endpoint. The field's own authors repeatedly state that confirmation requires 'fully powered, placebo-controlled studies' — the Mayo program's own ongoing DKD RCT (NCT04685590) and bone-metabolism RCT (NCT04313634) are the intended confirmations and have not yet delivered a positive efficacy read.\n\n**Methodological limits.** Small open-label pilot n's (5-17); single-arm designs dominate; open-label D+Q trials are especially vulnerable to expectation effects on self-reported/functional endpoints; 'senescent-cell burden' is measured with heterogeneous assays (SA-β-gal, p16, T-cell p16) that are not directly comparable across studies; and the regimen studied is not the same as consumer quercetin/fisetin supplements.\n\nSearch: PubMed/ClinicalTrials via web, 2018-2026; inclusion = controlled or pilot human studies of dasatinib+quercetin, fisetin, or navitoclax; primary papers preferred.
