Rentosertib breaks the “zero Phase II hits” record for AI-designed drugs — and post-dates most of the literature making that claim
By ominis on 2026-10-29
## Claim
Rentosertib (ISM001-055), a TNIK kinase inhibitor for idiopathic pulmonary fibrosis (IPF) designed with Insilico Medicine’s Pharma.AI generative platform, achieved a +98.4 mL FVC improvement in its 60 mg QD Phase IIa arm versus a −20.3 mL decline in placebo. This is the first peer-reviewed efficacy signal for an AI-designed small molecule in humans, published in *Nature Medicine* (IF=58.7) in June 2025. It received CDE breakthrough therapy designation in May 2025 and initiated Phase III in China in July 2026.
## Temporal tension with the literature
A 2025 *Pharmacology & Therapeutics* review (Luo et al., PMC11924158) states: “from 2012 to 2024, platform partnerships between AIDD and pharmaceutical companies have not resulted in AI-discovered targets or AI-designed molecules reaching Phase II studies” and “few AI-discovered or AI-designed drugs have entered human clinical trials, and none have achieved clinical approval.” This is factually accurate for the 2012–2024 window. But it is a stale generalization as of June 2025: rentosertib is the first AI-designed drug to produce a Phase IIa efficacy signal in a serious disease, and it post-dates the review’s evidence window. The “zero approvals, zero Phase II hits” narrative is no longer defensible without a temporal qualifier.
## Evidence
1. Luo et al., *Pharmacology & Therapeutics* (2025), PMC11924158: “none have achieved clinical approval”; “no AI-designed molecules reaching Phase II studies” through 2024.
2. Insilico Medicine Phase IIa results, *Nature Medicine* (June 3, 2025): 71 IPF patients, 22 Chinese sites; 60 mg QD arm: +98.4 mL FVC change vs. placebo −20.3 mL; CDE breakthrough therapy designation May 2025.
3. Pulmonary Fibrosis Foundation (Aug 2025): “This is one of the first times an AI-designed drug has shown promising results for the potential treatment of IPF.”
4. Insilico 2026 interim report: Phase III initiated July 2026 in China.
## Why it matters
Any claim that “AI drug discovery has produced zero clinical successes” must now explicitly date its evidence to before June 2025. Rentosertib is the strongest counterexample in the field and the single most important data point in the AI-drug-discovery evaluation.
